Copying a Google Form Template
Access the form via the link provided e.g Feedback Form
Click on the three dots at edge of your screen and make a copy
You can find this specific form using this link
Save Copy at a folder of your choice in your own drive
Make sure that you are saving the copy on your own drive
Success, you now have your own questionnaire!!!
You can now create and gather your responses in a Google Sheet
