CPCS and AVS

This guide will help you to refer patients to the new Community Pharmacist Consultation or Acute Visiting Service via SystmOne word referrals and send via NHS Mail

Please Note:

For CPCS referrals you should ask the patient to turn up to the pharmacy any time after 9am- Any Problems please speak to Jane Millward

For AVS you must contact the AVS first to see if they have capacity for the visit after 2pm before sending the phone number to call is 01743 454902

Open the Care Navigation Template

Using the Green Dot in the patients demographics box open the Care Navigation Template as shown below

Using the Referral Wizard

  1. Select the External Referrals Tab
  2. Tick the Box Signposted to Community Pharmacy or Signposted to AVS
  3. For CPCS Referrals - select Referral to CPCS. For AVS select and add a quick note for reason for visit and then select Referral to GP AVS

Confirming the Referral Details

The following Dialogue confirms your selection of referral details

1. (CPCS Referrals Only select the chosen pharmacy you do not need to do this for AVS as this is already pre filled in)- Select Address book

please note you can also use your Organisation Drop down box  (this list is a list of your favourite address in the address book)

2. Select Write Now

You do not need to fill in any more information in this screen, you may be propted to fill a referral ID (you do not need this and you can leave this blank)

CPCS Referrals Only (using the address book)

1. Select the search tab

2. Type in CPCS

3. Select Search

You will then be shown a list of the available pharmacies in the CPCService as shown below

4. Select the pharmacy you are referring the patient into

5.Press Select button at the bottom

 

Completing the referral form

The referral form will now have opened for you to complete. Most of the details needed are already pre-filled in for you.

1. Type in the reason for referral in the Symptom Group box as shown below. (there are symptoms for you to copy and paste into the box provided)

2. Once you have finished the referral letter select Save Final Version (you will not be able to edit this once you have saved the final version)

Once completed Confirm the Care Navigation Template by pressing ok (you will need to select Informal Referral- signposted to other agency as shown below

 

 

Emailing the referral

1. Select Referral Node from the clinical Tree

2. Right click on the referral you have just done

3. Select Send via NHS mail

4. Chose Existing Contact from the dialogue below

The address book will be displayed again and should have saved your previous searches for you to select the pharmacy of your/the patients choice if not:

1. Select the search tab

2. Type in CPCS

3. Select Search

You will then be shown a list of the available pharmacies in the CPCService as shown below

4. Select the pharmacy you are referring the patient into

5. Press Select button at the bottom

 

Confirming the email address

Confirm the email address in the question box below (this had been pre-configured for you)

1. Press YES

2. Type in the box please see attached referral

3. Press ok

This process is now complete. Make sure that you save the record
