LTC (differentiated conditions) review
This is a general guide and outline of principles for conducting a clinical/ therapeutic review for patients who have an existing diagnosis and may be starting or already on treatment.

Check the patient demographics (name, dob, address) and what long term conditions the patient has. This can be seen in the Patient Status Alerts. In this example the patient:
- Diabetic
- Obese
- Never-smoked

Ensure the patient status alerts correspond with the 'Problem List' and also the repeat medication template.

Check and update the recall to the next birth month as appropriate

Consider using the appripriate clinical view for the condition being reviewed. Not all views are available.
