CR4 cremation form
This guide is for medical practitioners completing the cremation form, digitally. It will need to be filled in, then exported and sent via email to the crematorium and/or funeral directors.
1. Create a new patient letter
Choose which document template to use
Fill in the whole document (cremation form). Some of the sections will be auto and pre-filled.
Ensure that you are able to upload and sign the document with your specific signature.
Save the PDF locally
Save the file for future editing as amendments may be required.
